These students are people we care about, so we have to do our due diligence. Please complete all these questions. If you answer "Yes" to any question, we need to know about ALL offenses, and for each conviction we need to know the date of conviction and disposition, regardless of the date or occurrence. Your answers will be verified with a background check. We know you care about kids too and understand why we have to ask!
Criminal offenses include felonies, misdemeanors, summary offenses, and convictions resulting from a plea of no contest.
Conviction is an adjudication of guilt and includes determinations before a court, a district justice or a magistrate, which results in a fine, sentence, or probation.
**We don't need to know about minor traffic violations, offenses committed before your 18th birthday which were adjudicated in juvenile court or under a Youth Offender Law, and any convictions that have been expunged by a court.
Professional discipline means the annulment, revocation, or suspension of your teaching certification or having received a letter of reprimand from an agency, board, or commision of state government
